Focus: Patients in need of referring physicians in PA
Goal: Make it easy for both patient and physician.
How: Physician Questionnaire, filled out online or by phone via volunteer and made searchable online by proximity and more.
Roles and Responsibilities:
- Outbound contact with physician groups/practices
- Database management/Data entry
- Direct candidates to Web-based on-boarding solution
